1.     That the deponent is the owner the vehicle  No __________________.

2.     That the letter pad & Form No 35 issued by ______________________________ Finance for cancellation of H.P. of the said vehicle has been lost somewhere an  are no traceable to the best of my efforts.

3.      That the above mentioned letter pad & Form No 35 not used by me for any purpose many registering authority anywhere in India till to date.

4.      That the deponent is in need of duplicate form No 35 and letter pad are genuine.

5.      That if my said statement will' be found false / begus in the future, then I shall be fully responsible for that
